Purpose:

Serves as liaison between St. Edmond's Home and various community agencies (Intermediate Unit, County Departments of Children and Youth, and offices of MH/MR, etc.). Participates as an active member of the IDT including assessments and program meetings. Serves as St. Edmond's representative for pre-admission/admission and discharge processes. Also provides direct supervision to St. Edmond's Home for Children's Family Caseworker.

Qualifications:

  • Master's Degree in Social Work is required for this position. Must be a licensed Social Worker.
  • Preferably have five years of experience working with children with ID.
  • Previous supervisory experience is a plus. This position supervises Family Supports Coordinator.

Responsibilities:

· General: Unusual incident reports (investigations, distribution), child abuse investigations (when necessary), attend safety review, human rights, and utilization review committee meetings. Maintain updated information of BSU and DHS case managers. Maintain up-to-date chronological records regarding all contacts. Monthly census.

· Application Process: Respond to inquiry/screen, send application when appropriate, collect medical, social, educational, psychological information, conduct tour of facility and meet with family.

· Pre-admission: Gather information to be reviewed by staff, set up pre-admission evaluation and inform staff, gather pre-admission evaluations from team, do pre-admission social assessment showing family's need to seek ICF/MR residential care for child and inform family of decision by letter.

· Admission: Guide family through process on the day of admission, apply for MA and SSI funding, inform CSS of admission, register child for school, register resident with appropriate BSU, maintain updated waiting list.

· Discharge: Work with BSU case manager to search for facilities, look independently for facilities/vacancies, send resident overview to prospective facilities, complete application for specific facilities, gather and send therapy evaluations, medical, educational, social and psychological information to new facility, arrange and accompany resident for a visit to facility for pre-admission evaluation, confirm funding for placement, arrange transfer of resident and his/her possessions, transfer SSI money and Representative Payee status, inform CSS of discharge, and do follow-up telephone call one week and one month after discharge.

· School: Act as liaison with all school personnel (letters, meetings, telephone calls) to problem solve and to coordinate IHP goals with those of IEP. Gather school program information for the IHP, inform QMRP of IEP meeting, track IEP (date of meeting, date we receive copy, pursue copy when one has not been sent to us.) Record Daily Medical information from schools and impart to St. Edmond's staff. Inform staff of school calendar and activities.

· Court: Act as liaison with DHS (phone and written reports), attend court hearings when invited- attend FSP meetings with lawyers, DHS caseworkers, client/family, and arrange supervised meetings with resident and family.

· IHP: 90 Day Note, annual summary for each resident, invitation to families, BSU, DHS. Send out copies of IHP to family and appropriate agency, attend IHP meeting, sign interpretation sheet, and confirm that Client Authorization and Resident's Rights forms have been signed.

· Funding: Apply for SSI for new residents. Apply for ICF/MR (MA) funding upon admission to St. Edmond's. Monitor certification/recertification of MA funding upon admission, after discharge from hospital, and on yearly basis. Maintain contact with funding agencies

· BSU: Register each resident with a BSU. Liaison with case manager, and give updated information about resident to the BSU as needed, with a view to eventual discharge. Invite case manager to the IHP meeting.
