PATH is seeking candidates with case management experience to join our South County Programs as Lead Case Manager. This position will be based out of our location in Long Beach and provide field-based services.

This position works primarily in the field throughout LA County's Service Planning Area (SPA) 7 with an assigned home office of our location in Long Beach. Cities encompassed by SPA 7 include: Artesia, Bell, Bellflower, Bell Gardens, Cerritos, City of Commerce, City Terrace, Cudahy, Downey, East Los Angeles, Hawaiian Gardens, Huntington Park, La Habra Heights, Lakewood, La Mirada, Los Nietos, Maywood, Montebello, Norwalk, Pico Rivera, Santa Fe Springs, Signal Hill, South Gate, Vernon, Walnut Park, Whittier, and others.

ABOUT THE PROGRAM
Our South County Programs comprise of both Time Limited Subsidy and Homelessness Prevention & Problem Solving Programs. Respectively, these programs provide temporary financial assistance and services to return people experiencing homelessness to permanent housing and temporary financial assistance and support to prevent people from falling into homelessness.

WHAT IS A JOB TITLE
As part of the street outreach team, the Lead Case Manager is responsible for supporting the Program Manager in the oversight of the program, including assisting with documentation quality oversight, reporting, and providing case management support to other program staff. Position Responsibilities:

  • Maintain 75% of the standard case load for the program.
  • Conduct daily street outreach to develop relationships and build rapport with clients in order to assist them with finding a home.
  • Conduct VI-SPDAT, (Vulnerability Index- Service Prioritization Decision Assistance Tool), on all new clients in order to connect them to proper intervention and services. Document results, complete match initiation form, and submit all paperwork to CES Coordinator.
  • Provide street-based case management services that are designed to prepare and connect homeless clients to long-term supportive services including:
    • Assistance with the establishment of personal benefits including (GR, SSI, TANF, Veterans Pension, etc.).
    • Assistance with obtaining various forms of identification including birth certificates and social security cards.
  • Meet with established clients on a weekly basis to review, evaluate, and support goal completion.
  • Respond within specific contract timeframes to requests for street outreach services and send follow-up information to the requestor and the supervisor.
  • Build relationships with prospective landlords and identify appropriate permanent housing options.

WHAT YOU BRING
We're looking for candidates that:

  • Demonstrated knowledge and experience with advanced and evidenced based interventions such as Harm Reduction, Motivational Interviewing, Critical Time Intervention, and Housing First.
  • Engagement and rapport building skills with hard-to-reach clients.
  • Knowledge of non-violent crisis intervention techniques.
  • Ability to work independently and as part the team.
  • Ability to exercise mature and professional judgment.
  • Good problem solving and conflict resolution skills.

PREFERRED QUALIFICATIONS

  • Two (2) years of experience in healthcare, case management, homeless services or working with vulnerable populations.
  • Written and spoken Spanish language proficiency.
  • Experience in street outreach with highly vulnerable populations, particularly those with health, mental health, and substance abuse issues.

MINIMUM QUALIFICATIONS

  • One (1) year of experience in healthcare, case management, homeless services or working with vulnerable populations.

MANDATORY REQUIREMENTS
Regardless of qualifications, candidates must:

  • Have employment eligibility verification
  • Have or be able and willing to obtain CPR/First Aid training
  • Provide proof of full COVID-19 vaccination
  • Successfully complete the following as a condition of hire:
    • Tuberculosis Test
    • Background Screening
    • Drug Test

DRIVING
Driving is an essential function of this position. To meet the employment requirements for this role, all candidates must:

  • Have reliable transportation
  • A valid driver's license
  • Proof of insurance and ownership for personal vehicles used during work duties
  • The ability to qualify for PATH's insurance coverage

COVID-19 VACCINATION
Effective 11/01/2021 PATH will require all employees to be fully vaccinated for COVID-19. All prospective hires will be expected to provide proof of full vaccination as part of the pre-employment credentialing process.
