Lubbock's fastest-growing and top chronic care management company is looking for an exceptional individual to help us with our increasing patient volume. This is a full-time office position that does not require face to face contact with patients. Responsibilities include, but are not limited to, contacting patients by phone to discuss their current medical condition and needs, scheduling phone calls for nurses, reviewing MD schedules for potential program patients, and documenting notes and information in our EHR.

Our unique approach to delivering exceptional patient care has helped us to continue to grow. Additional compensations opportunities include monthly bonuses and occasional overtime pay.

Requirements:

• A minimum of one year experience in a physician's office or similar setting is required.

Advance Care Management's ideal candidate is a team-player, punctual, and enjoys talking one-on-one with patients about their medical needs. Additional preference will be given to applicants who have experience using computers at work in addition to past CCM experience.

Job Type: Full-time

Pay: $32,000.00 - $38,000.00 per year
