Our Hospice team covers 8 counties in central Iowa and has various openings for growth!

Compensation: The minimum pay rate for this position is $33.15/hour. Starting pay rates will be based on years of experience.

Hours

Monday-Friday is Flexible but the possibility of the hours 10:00 am to 7:00 pm or 9:00 am to 6:00 pm

As an Admissions Nurse for our Hospice team, you will:

  • Traveling to CHLs and homes to see our hospice clients and their families
  • Complete initial, resumption, follow up, a significant change of status and discharge assessments including comprehensive head to toe assessment, HIS assessment, and all associated assessments with admission or discharge
  • Initiate an individualized plan of care, and update the plan of care for resumptions, significant change in status, and re-certifications.
  • Use health assessment data to determine nursing diagnosis
  • Obtain, implement, perform and maintain orders as received by physicians overseeing patient care services
  • Initiate appropriate preventive and rehabilitative nursing procedures. Administer medications and treatments as prescribed by the physician
  • Counsel the patient and family in meeting nursing and related needs
  • Provide health care instructions to the patient as appropriate per assessment and plan of care
  • Assist the patient with activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care.
