Job description

Join our dedicated team of health care professionals servicing adults and elderly living independently in their own homes. We are looking for RNs to provide skilled nursing in the Minneapolis area.

Job Duties

• Meet with clients one-on-one in their own homes
• Create and update the client's plan of care based on their desired goals and outcomes
• Perform skilled nursing tasks based on each client's plan of care
• Educate client on their specific needs to reach desired goals and outcomes
• Coordinate client cares with interdisciplinary teams when necessary
• Supervise Home Health Aide, PCA and Homemaking services when necessary
• Daily documentation of client visits using electronic health records system (some work can be done remotely from home allowing for more scheduling flexibility)
• Schedule your own client visits to be performed Monday through Friday (flexible start/end times, some of the work can be done remotely)
• Participate in nurse on-call rotation every eight weeks

Candidate Requirements

• Active MN RN License in good standing
• COVID Vaccination or Medical/Religious Accommodation
• Valid driver's license with good driving record
• Willingness to travel daily within your designated caseload area
• Have good organization, time management and be able to meet deadlines
• Utilize and navigate EMR system on company issued iPad
• Nursing experience preferred, but new grads encouraged to apply

Pay: $80,000.00 - $95,000.00 per year
