Job description

Seeking a Service Coordinator for our Senior Living Apartment Community to join our team! This position is responsible for coordinating programs and services to assist residents in maintaining the highest practicable level of independence and physical, mental and psychosocial well-being.

Essential Functions Include

· Assisting and educating residents and their families to acquire and utilize community services

· Promotes wellness activities for all residents

· Coordinates educational events for residents, families, and staff on available community resources

· Assists residents in building informal support networks with other residents, family members and friends

· Acts as a liaison between community agencies, service providers and residents

· Monitors the delivery of services to residents to ensure they are appropriate, timely, and satisfactory

· Coordinates volunteer support programs with service organizations in the community

· Maintains a resource directory of available community resources

· Maintain professional relationship with all residents in the housing community to identify areas of need

· Maintains resident files

· Submits all required reporting data within specified timeframes

· Networks with service providers to maintain necessary relationships and pursues avenues for additional services through private, local, state and federal resources

· Performs all other duties as assigned or directed

Qualifications

  • Bachelor’s Degree in social work - Preferred
  • Must have strong organizational skills and be deadline, detail and people oriented
  • Two or more years’ experience working with social service delivery, preferably to elderly population. Working knowledge of resources available in geographic location serving the community
  • Membership and familiarization with America Association of Service Coordinators helpful
  • Must be able to proficiently operate a computer, the Internet and basic office equipment
  • Valid driver’s license and reliable transportation required
