Position: Social Service Assistant/Unit Coordinator
Location: Lakeland Rehabilitation and Health Care Center, Effingham, IL 62401
Shift: Full-time
Preferred Social Service Assistant/Unit Coordinator Experience/Requirements:
- Assist in Develop, implement, evaluate, and direct the social service programs of the facility
- Participate in community planning related to the interests of the facility and the services and needs of the resident and family
- Assist in maintaining community relationships to help address resident concerns as appropriate
- Assist & perform routine tasks with patient admissions, care plans and discharge planning
- Maintain flexibility to address resident needs as they arise, multi-tasking is key!
