Job description
MJHS Home Care provides advanced clinical services and emotional support to patients recovering from illness, injury or surgery. With a comprehensive range of services supported by an integrated health system, patients can recover in the familiar surroundings of their home.
- Assesses patient psychosocial issues to ensure adequate delivery of home care services in order to meet agency goals and required objectives
- Provides supportive counseling
- Provides concrete services and related counseling
- Conducts Home visits in a timely manner
- Coordinates activities with other health team members to ensure optimal patient care and communication of pertinent findings
Field based position will coverage: Brooklyn, Manhattan, Queens and Bronx
- Licensure by New York State Education Department (LMSW or LCSW)
- MSW from an accredited College or University
- One year of professional Social Work experience in an acute care or social service agency
- One year of home care experience preferred
- Valid driver license and car
