Somerset Gardens in Plainview, NY is hiring for a FT Case Manager. The Case Manager Is the liaison between the residents and their families. The primary duties includes maintaining of all residents files, including Personal Date Sheet, Initial Assessments, Medical Evaluations, Case Management Assessments, Incident reports and other pertinent information .The requirements are Master’s Degree in social work from an accredited college or university; or Bachelor’s Degree from an accredited college or university with major work in human resources or service delivery and one (1) year of full-time experience in the provision of services to a dependent adult population; or An Associate Degree from an accredited college or university with major work in human resources or service delivery and three (3) years of full-time experience in the provision of services to a dependent adult population. Approval from New York State Department of health as a Case Manager. Competitive salary range between 58,500-63,500 Please send resume EOE
